A Note From Lisha, Director and ACM Golden Horizons

I’m Lisha Vandersteen, Director of ACM’s Golden Horizons Adult Day Center. What I love most about this work is helping families move from fear, exhaustion, and uncertainty to feeling supported, understood, and hopeful again. I have seen firsthand how the right environment, compassionate dementia-informed care, meaningful engagement, and consistent routines can completely change not only a participant’s quality of life, but the well-being of the entire family.

One story that stays with me is a caregiver who arrived overwhelmed, exhausted, and unsure if anyone could truly connect with her loved one anymore – and within weeks, she shared that for the first time in a very long time, her loved one was smiling again, participating, and looking forward to coming each day. That is why this center matters so deeply to me.

Originally opened in 2017, Golden Horizons became part of Aging Care Matters in 2025 when the previous owner retired, and ACM was honored to continue the center’s long-standing commitment to older adults and caregivers in the Durham community. Golden Horizons provides compassionate adult day care, dementia support, social engagement, and caregiver respite in a safe and welcoming environment.

What makes Golden Horizons distinctive is its larger 28-person capacity, multiple activity and quiet spaces participants can comfortably explore throughout the day, and its long-standing presence serving Durham families since 2017.

Miriam has helped me so much around navigating how to care for my in-laws. We moved them from out of state, found a house in 55+ community, she helped with choosing doctor offices, and what more we can do to help with transitioning to NC. When my MiL had scheduled shoulder surgery, she had a bad reaction to the anesthesia which made her have delirium, Miriam helped navigate which Rehab/nursing home care, then which home PT/OT/nurses, AND which home care companion companies! Aging Care Matters is the most amazing facility. I wouldn't even call it an Adult Day Center. It is more like family. I believe Carla and her staff truly care about every person there. I am incredibly grateful for the love and support provided by Carla, Stavie, Sydney, Wyatt, and the entire amazing care team at Aging Care Matters. Aging Care Matters Adult Day Center – Golden Horizons conveniently serves families throughout Durham and the greater Triangle area with easy access from major roadways and surrounding communities. Typical drive times include:

  • Durham — 5–20 minutes
  • Chapel Hill — 15–25 minutes
  • Research Triangle Park — 20–30 minutes

Many families appreciate Golden Horizons’ convenient Hillandale Road location near medical offices, Duke-related services, and major Durham roadways, making daily drop-off and pickup easier to incorporate into caregiving and work routines.

Families appreciate the center’s simple and stress-free drop-off process using our convenient side entrance and roundabout-style pull-up area. There is no need to park or leave your vehicle – simply pull up, and our staff will warmly greet your loved one at the car, escort them safely inside, and assist again at pickup to help make the caregiving day just a little easier.

A Typical Day At Our Wake Forest Center

A typical day at ACM’s Aging Care Matters Adult Day Center – Golden Horizons begins with warm greetings, coffee, conversation, music, and helping participants settle comfortably into the day. Throughout the morning and afternoon, participants enjoy structured cognitive engagement, movement, singing, dancing, games, creative art activities, exercise, shared meals, laughter, and meaningful social connection designed to support emotional well-being, routine, and cognitive stimulation in a safe and welcoming environment.

What makes Golden Horizons especially unique is the creativity and depth of its programming. Under Director Lisha Vandersteen, participants have explored detailed educational and interactive programs focused on every state across the U.S. and most recently an “80 Days Around the World” experience introducing participants to different countries, cultures, foods, music, and traditions. Families often tell us they are amazed not only by the level of engagement and stimulation their loved ones experience, but by the joy, laughter, confidence, and sense of purpose they regain through the center’s programming and relationships.

Memory Care & Dementia Support At Our Wake Forest Location

At ACM’s Aging Care Matters Adult Day Center – Golden Horizons, dementia support is deeply rooted in compassion, relationship-building, and understanding the unique experience of each individual living with cognitive change. Our team receives training in the Positive Approach to Care philosophy developed by Teepa Snow and supported through education from the Snow Foundation, helping staff better understand communication changes, behavioral expressions, sensory needs, and effective dementia care approaches that reduce fear, frustration, and resistance.

Golden Horizons’ larger layout with multiple activity and quiet spaces allows participants to move comfortably throughout the center based on their interests, energy level, and need for stimulation or calm. Structured routines, music, movement, creative engagement, hydration, familiar staff relationships, and lower-stimulation afternoon programming are intentionally designed to support individuals experiencing memory loss, confusion, anxiety, sundowning, or cognitive decline while helping families feel confident their loved one is cared for with dignity, patience, and respect.

Meals and Nutrition

Meals at ACM’s Aging Care Matters Adult Day Center – Golden Horizons are designed to provide nourishment, comfort, routine, and meaningful social connection throughout the day. Participants enjoy morning snacks, lunch, and afternoon snacks together in a relaxed and welcoming environment that encourages conversation, hydration, dignity, and engagement. One favorite tradition at Golden Horizons is “Fish Fridays,” when many participants look forward to enjoying fish meals from Captain D’s together, adding another layer of familiarity, fun, and community to the week.

Our meals and snacks are reviewed and approved by a licensed dietician, and ACM carefully follows physician instructions and family guidance regarding diabetic needs, low-sugar or no-sugar diets, soft-food diets, allergies, food sensitivities, and cultural or religious dietary preferences. Staff work closely with families to help ensure meals are both safe and enjoyable while supporting each participant’s overall wellness and quality of life.

Pricing And How To Apply

ACM’s Golden Horizons adult day care daily rate includes a full day of compassionate supervision, dementia-informed support, social engagement, meals, snacks, structured activities, medication reminders, and caregiver respite in a safe and engaging environment. Our rate is $112 per day for participants attending one to two days per week and $100 per day for participants attending three or more days per week, offering families one of the most affordable long-term care and dementia support options available.

Payment is made weekly based on scheduled attendance days, and if a participant misses a scheduled day, families may make up that day within the same calendar month when space is available. Families may pay by check, ACH, or credit card, though a 3% processing fee is added to credit card payments.
