Address: 1106 Hillandale Road, Suite C, Durham, NC 27705 ACM’s Golden Horizons Adult Day Center combines compassionate supervision, engaging activities, and dementia support in a welcoming environment with a 28-person capacity and several current openings for families seeking daytime care and respite support. Mornings at our Centers begin with warm welcomes, coffee, conversation, music, light movement, and structured activities that help participants feel calm, connected, and engaged from the start of the day. Midday includes shared meals, cognitive and social activities, laughter, games, creativity, and meaningful moments of connection, while afternoons focus on gentle engagement, relaxation, snacks, music, and preparing participants for a successful transition home. One recent moment captured exactly what ACM is about: after role-playing Positive Approach to Care techniques during a staff training, a team member used those same approaches later that day to help a participant who was struggling to stand from a chair. Instead of fear or frustration, the participant responded with calm, trust, and success — a reminder that compassionate, relationship-centered dementia care can completely change how a person feels in a difficult moment. ACM’s adult day care daily rate includes a full day of compassionate supervision, meaningful engagement, and caregiver support in a safe, structured environment. P Participants enjoy morning coffee or tea, morning and afternoon snacks, lunch, structured activities with supplies, social engagement, basic health monitoring, medication reminders, and supportive staff supervision throughout the day. Our adult day centers are open Monday through Friday from 7:30 AM–5:30 PM. ACM also provides complimentary care management consultation and guidance to all current adult day care participant families, helping caregivers navigate aging, dementia care, resources, and long-term planning with expert support. Late pickup fees apply after scheduled closing hours. Adult day care programs in North Carolina generally fall into two categories: social model and health model. Typical adult day care costs in North Carolina range from approximately $80 to $120 per day, making it one of the most affordable long-term care and dementia support options available. Families commonly pay for adult day care through four primary pathways: private pay, and long-term care insurance for eligible individuals. ACM also helps families understand available benefits, reimbursement options, and community resources that may assist with the cost of care. At this time, ACM does not provide transportation services. Many families partner with home care agencies, family members, medical transportation providers, or community transportation resources to assist with arrival and pickup. We recommend comfortable clothing, a labeled easy-to-change set of clothes, any needed incontinence supplies, and clearly labeled medications if medications are taken during program hours. All medications must remain in their original prescription bottles for safety and compliance purposes. At this time, ACM does not accept Medicaid or Veterans benefits directly for payment. Families commonly pay through private pay or long-term care insurance, and our team is happy to discuss potential reimbursement options and resources. ACM supports many individuals living with mild to moderate dementia and some individuals with more advanced cognitive decline depending on mobility, safety, and care needs. Because we are a social model adult day program, each participant is individually assessed to ensure we can safely and compassionately support their needs. Resistance to attending adult day care is very common in the beginning, especially for individuals living with dementia, anxiety, or changes in routine. Consistency is extremely important, and many families find it can take up to two months before the routine feels fully comfortable and familiar. We encourage families to use short, reassuring phrases such as “You’re going to spend time with friends today,” “They’re expecting you,” or “I’ll be back to pick you up later,” while avoiding overwhelming explanations or asking permission in ways that may increase anxiety. ACM is generally open Monday through Friday year-round with closures for select major holidays. Families receive advance notice regarding holiday schedules, optional closure days, weather-related closings, or modified hours.
