Our Aging Care Matters care management and administrative office hours are: Monday – Friday: 9:00 AM – 5:00 PM

During office hours, families and professionals can contact our team regarding:

If you contact us after hours, please leave a message and a member of our team will respond as soon as possible during the next business day.

After-Hours & Emergency Communication

For active care management clients, limited after-hours and holiday support may be available for urgent situations involving significant changes in health, safety, hospitalization, discharge needs, or caregiving crises.

Please understand that after-hours, evening, weekend, and holiday care management services are billed at higher rates than standard business-hour services due to the urgent and non-scheduled nature of the support provided.

Examples of situations families may contact us about after hours include:

  • Hospitalizations or emergency room visits
  • Unexpected discharges
  • Sudden decline or confusion
  • Caregiver emergencies
  • Urgent placement or safety concerns

However, Aging Care Matters is not an emergency medical service.

If there is:

  • Chest pain
  • Difficulty breathing
  • A serious fall or injury
  • Stroke symptoms
  • Loss of consciousness
  • Immediate danger to self or others
  • Any life-threatening emergency

Call 911 immediately.

One of the most common after-hours situations families contact us about is uncertainty after a hospitalization or sudden change — families are scared, exhausted, and unsure whether the situation requires emergency intervention, urgent medical follow-up, or simply guidance about next steps. What we most wish families understood is this:

Your Privacy Matters

We understand the information families share with us is deeply personal and often emotionally sensitive.

Aging Care Matters:

  • Does not sell or share your information with marketing lists or outside advertisers
  • Uses secure, HIPAA-aware communication and documentation practices
  • Collects only the information needed to respond appropriately to your inquiry
  • Limits access to your information to authorized team members involved in supporting your situation
  • Uses submitted information only for communication, care coordination, and providing requested resources or services

If you contact us and later decide you no longer wish to receive communication, you can request removal from future emails or outreach at any time.
