• Home Care Services – Help to determine the types of services that are appropriate for the client and assisting with those services.

• Medical Management –Arranging and attending doctor appointments and other health care providers (i.e. physical therapist). We facilitate communication between providers, the client, and their family members. 

• Entitlements – We provide information on Federal and State entitlements and connect our clients and/or families to local programs.

Bill Pay & Home Organization – Reviewing or overseeing bill paying or consulting with accountant or client’s representative who has General Power of Attorney to act on behalf of the client.

Housing Assistance – We help our clients and their families select and evaluate appropriate levels of care and residential options. (ie. assisted living, skilled nursing facility, etc.)

Financial Assistance- Reviewing or overseeing bill paying or consulting with accountant or client’s representative who has General Power of Attorney to act on behalf of the client. 

Safety & Security – Monitoring clients at home, recommending technologies that can add to security and safety. In addition, we observe changes and potential risks of exploitation or abuse.

Crisis Intervention Services

Emergencies are bound to happen, and when they do, Aging Care Matters has resources to help you and your loved ones. Our team has the ability to organize and manage hospitalizations, rehabilitations stays, and in-home health care.

Ensure that your loved one is consistently cared for by signing up for our Care Visit Program. This monthly subscription program includes:

• Weekly Call

• One Monthly one-on-one Visit

• 24 /7/365 Access to your Care Manager

Our Call Care Program helps to ensure consistent communication between your loved ones and our care management team. Call Program Options:

3  Days per Week Check-in Call
